A pulmonary mass and hyperviscosity.

چکیده
A 69 year old woman was referred to hospital because of a persistent dry cough and dyspnoea. She has never smoked and was taking captopril for hypertension and thyroxine for hypothyroidism. At age 20 she developed pulmonary tuberculosis which was treated with a left artificial pneumothorax. A few years later during the course of investigation into infertility she was found to have tuberculous endometriosis and salpingitis. This was treated with streptomycin injections and isoniazid. A large mass measuring 10 cm × 9 cm was seen on chest radiography (see figs 1 and 2). Computed tomography of the thorax demonstrated a solid, partially calcified 10 cm mass situated posteriorly in the left upper lobe. In addition there was loss of volume and concentric pleural thickening in the left hemithorax, probably as a result of the previous artificial pneumothorax. No metastases were evident. At bronchoscopy irregular mucosa was noted in the left upper lobe bronchus. A biopsy specimen of this showed abnormal lymphoid tissue. Staining of sputum for acid-alcohol fast bacilli and subsequent mycobacterial cultures were negative. In the meantime the patient had developed marked hypertension, spontaneous bruising, and epistaxis. Fundal haemorrhages were present. Blood tests revealed a plasma viscosity of 5.3 mpa/s (normal range 1.5–1.72), haemoglobin 114 g/l, white cell count 3.9 × 10/l, platelets 230 × 10/l, globulin of 81 g/l, and an IgM monoclonal band of 42 g/l. Renal function was normal.

A clinical picture of pulmonary embolism revealing light‐chain myeloma
We are highlighting on the particularity of a clinical picture of pulmonary embolism revealing light-chain myeloma in a 56-year-old male patient. Myeloma remains a rare affection. Even though its revelation through pulmonary embolism remains rare, it can be explained by hyperviscosity syndrome accompanying it.
متن کاملRituximab for treatment of rheumatoid hyperviscosity syndrome: Pulmonary arterial hypertension and post- partum cardiomyopathy cases
Hyperviscosity syndrome (HVS) is a rare manifestation of rheumatoid arthritis with few reported cases. Although plasmapheresis is effective as the first line treatment in life threatening manifestations such as pulmonary hypertension, stroke, retinal hemorrhages and congestive heart failure, data on effective maintenance immunosuppressive therapy is lacking. HVS is characterized by abnormal imm...
متن کاملCentral retinal vein occlusion in a young adult Case report.
PURPOSE To report the case of a 48-year-old man with unilateral central retinal vein occlusion. METHODS The clinical, hematologic and hypercoagulability evaluations of the patient were thoroughly and specifically carried out. RESULTS The central retinal vein occlusion was of nonischemic type and was associated with polyglobulia, hyperleukocytosis, hypercholesterolemia, multiple dental foci,...
متن کاملPulmonary hypertension secondary to hyperviscosity in a patient with rheumatoid arthritis and acquired von Willebrand disease: a case report
INTRODUCTION Acquired von Willebrand disease is initiated by autoantibodies and hyperviscosity syndrome caused by a massive polyclonal hypergammaglobulinemia. Acquired von Willebrand disease associated with autoimmune disease in addition to pulmonary hypertension during emergency room presentation is a rare condition. To the best of our knowledge, this is the second case reported in the literat...
متن کامل18FDG PET/CT in pulmonary carcinosarcoma and brain metastasis
Carcinosarcoma is a rare type of cancer that is composed of a mixture of sarcomatous and carcinomatous elements. Pulmonary carcinosarcoma has a 25% five-year survival rate with a prognosis poorer than other non-small cell lung carcinomas. Herein, we report a case of pulmonary carcinosarcoma and its 18F-FDG PET/CT findings. A 61-year-old male patient presented with brain symptoms, including hea...
متن کاملUnsteady flow and mass transfer in models of stenotic arteries considering fluid-structure interaction
In this work the unsteady non-Newtonian blood flow and mass transfer in symmetric and non-symmetric stenotic arteries are numerically simulated considering the fluid-structure interaction (FSI) using the code ADINA. Blood with hyperviscosity syndrome is considered and hyperelastic Mooney–Rivlin model is used for the compliant arterial wall. The inlet boundary condition of imposed velocity or pr...
